Helping hero bugged

I certainly haven’t but I’ve been levelling in caves, grouped, solo, I’ve sold stuff to shops, bought stuff from shops, used traders, used teleports. Generally played the game as it should be played.Has anyone considered/tested what "gives" you the quest. It might not be purely time based, could have other hidden requirements perhaps?
Unless of course we need to be speaking to specific NPC in specific towns to trigger it. Which would just be a bit nonsense trying to find that.

Mine just resets everyday about 12.20.
If I miss one I can usually do it the next day up until 5pm ish and will still get it reset again about 30mins after completed it

They are extremely good and I would hate to miss them. I hope you can resolve it.
Theres an issue somewhere and under certain conditions it just seems to stop refreshing and seemingly required a reboot to restart it all again. i know certain heroes quests require certain things to get the quest. Kill meter or id scroll an item etc... it would be good if we knew all the triggers or if there was a master reset npc your char could use to reset it all if it seems to bug again.
